Patient Guidelines for home visits

New patients will have a Virtual Visit intake prior to your home visit (included in first visit fee.)

All patients will have their temperatures checked at their door and be asked to wear a mask that covers their nose and mouth.

All patients will be asked to complete a COVID-19 screening form before each home visit
